This will be a Patient sign up event that will include the following services:

• Information about how to apply for the IL medical cannabis program

• Application assistance team will be present if you are ready to apply

• Doctor onsite who can certify for your medical cannabis card

• Patient Consultation Services (30 & 60 minute segments available

• Free samples of CBD topical balm

Costs for each service:

• IL State Medical Card Fees: $100 for 1 year card, $200 for 2 year card, $250 for a 3yr card

• Doctor’s Certification & Initial Consult $300

• Patient Consult Services $60 per hour/$30 per half hour

Patient will need to bring proof of residency: a drivers license, state ID, or Passport +2 bills in your name
